• 16-55 years old $2400/year
• 55 years old plus $3300/year

• Unlimited appointments; in-home and telehealth.
• Unlimited call/text/portal messaging.
• Appointments within 24 hours.
• Same-day nurse triage.
• Same-day return phone calls/text messages.
• Mobile Phlebotomy, EKG, Xrays/Ultrasound
• Collaboration with local providers/specialties.
• Easy to navigate Telehealth platform.
• Easy patient portal messaging.
• Transparent portal sharing of appointment notes, labs, imaging, etc.
• Promotion of discussion between the member and provider, education, and ability to ask questions at the appointment and thereafter via phone, text, and/or portal messaging is encouraged. 
• No additional cost or copayment for in-home, telemedicine, or phone consultations.
